
2025 Treasure Valley Community Health Needs Assessment Survey
About the Community Health Needs Assessment
Every three years, WICHC, a collaborative composed of public and private partners launch their Community Health Needs Assessment (CHNA) alongside its health partners such as St. Luke's, Saint Alphonsus, Central District Health, Southwest District Health, Weiser Memorial Hospital, Terry Reilly, and United Way of Treasure Valley to better understand the health needs and priorities within its 10-county footprint (Ada, Adams, Boise, Canyon, Elmore, Gem, Owyhee, Payette, Valley, and Washington).
To support WICHC’s CHNA, you can get involved by supporting its data collection phase: March - May 31st. You can take the community survey, participate in a focus group, or host a focus group in your community.
Get Involved: Your Voice Matters

Distribute the survey among your community. The survey takes no more than 10 minutes to complete and is strictly confidential. Participants will enter to win a $20 gift card and can expect to be notified by early June (up to 30 gift cards will be randomly selected from survey participants). For additional languages, or to request paper copies, contact survey@metop.io.
