Phase 2: Action Plan Development

Using the Qualitative Needs Assessment as a guide, WICHC and the Elmore County Health Coalition created a four-item action plan to achieve optimal health and collaborative services.


The four-item action plan

1. Implement a Community Health Worker (CHW) program

2. Utilize Community Health Emergency Medical Services (CHEMS, Community Paramedic)

3. Develop a mobility consortium

4. Increase the accessibility of trails and open spaces


Community Health Worker (CHW) Program

Community Health Workers (CHW) serve as a link between multiple health services and other resources in a community and the citizens. Many citizens may not know all of the resources available to them; a culturally competent CHW will be the connective liaison to bring those resources and the patient together.

Community Health EMS (CHEMS) is a revolutionary reaction to preventable ambulatory response calls. CHEMS response will address the root causes of many preventable calls with a patient-centered outlook, such as directions on using medication, post-discharge follow-ups, and chronic disease management, such as diabetes.

Elmore County does not have the transportation resources of larger demographic areas, such as Ada County. A mobility consortium will provide collaborative resources to improve the walkability and the built environment of Elmore County by participating in projects and sustainability practices to positively impact the county for pedestrians.

Mobility Consortium

Elmore County is home to acres of beautiful land. In order to promote physical activity and active transportation, conserved land and additional trails will be created with an emphasis on public engagement to increase the knowledge of trail use and best practices. Community open spaces will also be created in order to provide a space for community activities.
